3/11/2024 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Broadnax. When QMHPA arrived to her home, Ms. Broadnax was ready for the session evident by her pacing in front of her apartment building. Ms. Broadnax verbalized to QMHPA that she was in a “pissed off mood” and that she was about to “go off “on someone. QMHPA informed her that the session would address decreasing anger and anxiety. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. QMHPA observed as she pulled up to the home that Ms. Broadnax was walking back and forth in the hallway of her apartment building. QMHPA asked Ms. Broadnax what had triggered her anger outburst. QMHPA redirected Ms. Broadnax for breaking things in her home. QMHPA directed Ms. Broadnax contact her psychiatrist for an immediate evaluation/appointment regarding her psychotic symptoms and a possible change in her medications. QMHPA guided Ms. Broadnax through a strategy (guided imagery) to help her manage her response to stressors and anger. QMHPA assisted Ms. Broadnax in recognizing that she needed to apologize to those that were in her home this morning in a constructive and respectful manner for her negative and challenging behaviors. QMHPA assisted Ms. Broadnax in contacting the psychiatrist office and scheduling an emergency appointment to be seen. Ms. Broadnax stopped pacing when she saw QMHPA pull up. Ms. Broadnax stood standing in the hallway until QMHPA approached her and verbalized her fear that she had gone too far with her dad’s girlfriend. Ms. Broadnax’s father informed QMHPA that if Ms. Broadnax’s psychiatrist did not do better with the medicine she had prescribed her then she would have to move out of the home because she is displaying behaviors that are not acceptable in the home and he knows that their relationship has not been all the way good but she cannot be slamming door breaking items in the home and just being disrespectful. Ms. Broadnax apologized to her father for her hostile and disrespectful behaviors. She stated to QMHPA that people just gets on her nerves and they wonder why she is always in the streets.
